Join Our Team as a Support Facilitator (Home Care) – Make a Real Difference Every Day! Are you passionate about helping people live their best lives? Staffordshire County Council’s Provider Services team is looking for dedicated Support Facilitators to deliver person-centred reablement care in the community. You’ll be part of an innovative and empowering service that supports individuals to thrive in their own homes.Our service runs 7:00 AM to 10:00 PM, and we need team members who can start early and work flexibly to meet the needs of those we support. Applicants must live within 30 minutes of our base location ST5 1BL&nbsp;to ensure timely response and continuity of care.This role requires travelling within the community, primarily Newcastle under Lyme Borough, and so a full UK driving licence and access to your own vehicle is essential.We have a part time position available with a range of shifts between 7am and 10pm, which will include weekends. There is a business need for the early shifts to start at 7am. We now have one great opportunity to join the county’s high performing and citizen-focused Neurodivergent team, as part of the Health and Care Directorate at Staffordshire County Council.The role of the Intensive Support Worker has been created specifically for the Neurodivergent Team, in order to develop the skills and abilities of Neurodivergent Adults within Staffordshire. This role provides practical hands-on direct community work.You'll work within a new bespoke team, set up specifically to work with Neurodivergent Adults to develop skills and independence and to lead fulfilling lives and with the Consultant Social Worker for Autism, who sits within the Neurodivergent Team, for bespoke advice guidance and support.&nbsp;Location: This is a hybrid role, with community visits required on up to 5 days of the week, alongside regular office presence. Providing crisis intervention and working collaboratively with social workers to support children in need, children on child protection plans and children in care.You'll be expected to undertake flexible, creative and imaginative approaches to offering support to children, young people their families and carers whilst underpinning our restorative approach.This may include being available outside of traditional working patterns including evening and weekends to provide support to children, families and carers when it is needed.Main Responsibilities Our Family Practitioners are key workers for the children and families we support.
