We now have one great opportunity to join the county’s high performing and citizen-focused Neurodivergent team, as part of the Health and Care Directorate at Staffordshire County Council.The role of the Intensive Support Worker has been created specifically for the Neurodivergent Team, in order to develop the skills and abilities of Neurodivergent Adults within Staffordshire. This role provides practical hands-on direct community work.You'll work within a new bespoke team, set up specifically to work with Neurodivergent Adults to develop skills and independence and to lead fulfilling lives and with the Consultant Social Worker for Autism, who sits within the Neurodivergent Team, for bespoke advice guidance and support.&nbsp;Location: This is a hybrid role, with community visits required on up to 5 days of the week, alongside regular office presence. There is the flexibility to work from home if desired, around the visits and office presence. This is a permanent role. The team is based between Stafford and Burton with a dual office base.&nbsp;Main Responsibilities As an Intensive support worker you will:<ul><li>Complete direct work in the community with the citizens of Staffordshire, building up their skills and independence for specific goals and outcomes in line with support plans created by Social Care assessors.</li><li>Build up good local community knowledge using available resources for the citizens of Staffordshire.</li><li>Promote outcome-focused working with the citizens of Staffordshire.</li><li>Work alongside a small team of Social Workers, Social Care Assessors and other intensive support workers to support Neurodivergent adults achieve their outcomes.</li><li>Contribute towards case responsibility and management (working alongside a Social Care Assessor / Social Worker).</li><li>Working under the provisions of the Mental Capacity Act (2005) and Care Act (2014).</li><li>Record of all case management and key decisions on the case recording system.</li><li>Contribute towards managing risk appropriately with the allocated Social Care Assessor.</li><li>Actively participate in quality assurance processes, including peer and reflective supervision, whilst using research findings, service principles and case discussion with peers to inform and develop your professional practice.</li><li>Participate in development and training opportunities to progress professional development.</li></ul>The Ideal Candidate For this role you'll:<ul><li>Have a passion for direct working with individuals, within the community&nbsp;</li><li>Have experience of working with individuals with neurodiversity and their families.&nbsp;</li><li>He motivated by the purpose of your role – to help and support others. &nbsp;</li><li>Be a real team player, you’ll be able to flex to the needs of the team.&nbsp;</li><li>Have a proven track record of a person centred, and asset focused outlook and you’ll have high professional standards.&nbsp;</li></ul>“We’re happy to talk flexible working”&nbsp;Don’t feel you meet all the requirements? Where you can make a difference to the lives of children and their families?&nbsp;We are looking for two enthusiastic Family Practitioner to join our team in Stafford.You'll play a key role in working with children and families to support children to remain within their families. Providing crisis intervention and working collaboratively with social workers to support children in need, children on child protection plans and children in care.You'll be expected to undertake flexible, creative and imaginative approaches to offering support to children, young people their families and carers whilst underpinning our restorative approach.This may include being available outside of traditional working patterns including evening and weekends to provide support to children, families and carers when it is needed.Main Responsibilities Our Family Practitioners are key workers for the children and families we support.
