In this role you will work with adults in our local community. You will be responsible for completing Care Act assessments, undertaking safeguarding enquiries, facilitating and reviewing care packages, alongside a range of other responsibilities associated with supporting the delivery of adult social care services.

All about you

You will:

• Be registered with Social Work England.
• Have successfully completed your ASYE.
• Have a good understanding of current legislation, guidance and best practice relating to adult social care and mental health, including the Care Act, Mental Capacity Act, Mental Health Act and safeguarding responsibilities.
• Be committed to strengths-based, person-centred practice.
• Be able to work effectively within a multidisciplinary environment.
• Hold a full driving licence and be able to travel across the city and surrounding areas as required.
• Be committed to maintaining your professional development and continuous learning

All about our Team

At Peterborough City Council, our Adult Mental Health Team is dedicated to supporting adults with mental health needs to live safe, independent and fulfilling lives within their communities.

Our team is co-located with Cambridgeshire and Peterborough NHS Foundation Trust (CPFT), giving social workers the opportunity to work alongside mental health nurses, psychiatrists, psychologists, occupational therapists, support workers and other health professionals as part of a fully integrated multidisciplinary team. This close partnership enables timely decision-making, collaborative risk management, and joined-up support for the people we serve.

Working within this integrated environment, you will bring a social care perspective to mental health practice, undertaking Care Act assessments, safeguarding enquiries, strengths-based interventions, discharge planning, securing joint health funding and care planning, while contributing to holistic assessments and recovery-focused support plans.
