Case Management
Case management is split into two teams who cover 8 neighbourhood areas, which is part of the way we are organised in Hackney as we together meet the health and social care needs of individuals with our partners. The team completes initial assessments for people first coming into adult social care, as well as reviews and re-assessments for those with more complex needs. With a strong focus on practice and legal literacy, the team seeks to enable people to be as independent as possible with the breadth of assets and community opportunities available in Hackney.
Safeguarding Adults and DoLS Service
With a focus on keeping people safe, and promoting recovery and resilience from abuse, the Safeguarding Adults and DoLS Service focuses on the outcomes for an individual. Making safeguarding personal is the ethos of what we strive to achieve. The team is the first point of contact for the majority of concerns, as well as holding some cases. Human rights is central to practice and the service includes the Deprivation of Liberty Safeguards and management of this statutory area.
Integrated Discharge Service
The Integrated Discharge Service assesses Hackney residents, whilst in hospital across all London Hospitals and beyond, for the most appropriate and safe discharge destination and care and support plan. We focus on safe discharges that respect the legal rights and expressed wishes of all individuals. The team works in partnership with the multi-disciplinary teams within hospitals and attends multi-disciplinary meetings on their aligned wards. The service is inclusive to those in acute settings who are assessed as having a learning disability, mental health diagnosis, or dementia. It also supports residents with no recourse to public funds who have eligible care and support needs. The service operates seven days a week, and staff are invited to contribute to weekend working if they so wish. Community Reviews are completed collaboratively with the resident, family members, carers, and other involved professionals, to ensure that their long-term care and support needs are assessed and that any newly arising concerns or needs are addressed.
We are a team of Social workers, Senior Social workers, Senior OT and occupational therapists and Review Officers.
Access &amp; Duty
The Access and Duty Team serves as the initial point of contact for adults in Hackney who require intervention, advice and signposting from Adult Social Care. Our primary responsibilities include screening all incoming referrals and correspondence. We also respond to critical needs, risks, and emergencies for open cases, including unplanned reviews. The social workers and review officers undertake Care Act assessments for new referrals that have been screened and identified as requiring a Care Act assessment. The assessments are undertaken in the community with the resident and their respective carers.
We are a team of Senior Social workers, Social workers, Review officers and Information officers.

Lead on all day-to-day operations of the historic landscape of Birkenhead Park through management of the estate management team and visitor services team, with support from Friends groups and other organisations, ensuring that the park is well-used by stakeholder groups and events, while it remains in the highest quality state of maintenance of a Grade I listed historic park, and an aspirant a UNESCO World Heritage Site. In close collaboration with the World Heritage project team, deliver and implement the key objectives as set out in Birkenhead Parks Management Plan relating to its development including income generation, landscape changes, visitor services, and planting schemes.
About the candidate:
We would like to hear from you have HNC/HND in the Park Management Sector or equivalent qualification and experience along with understanding of heritage, conservation, land management and listed garden requirements. You should have the ability to form, build and maintain strong, effective working partnerships with other Council teams, the local community and external agencies.
Also required is strong networking and stakeholder management skills including presenting reports to committee and addressing public meetings with knowledge and understanding of World Heritage Sites and their management requirements.

Positive Change Service – the domestic abuse intervention service for LBTH Children’s Social Care – is seeking a permanent Lead Safety Worker to work with perpetrators of domestic abuse. This post is specified for a male worker as per an occupational requirement to enable us to use mix-gender co-facilitators in our groupwork with fathers.Positive Change Service delivers psycho-educational interventions with perpetrators, survivors and children impacted by domestic abuse. We are a creative, supportive and motivated team, working for systems change alongside personal change, to improve the lives of families in Tower Hamlets. The service is part of the Children’s Social Care, MASH and Assessment &amp; Intervention Service. The Housing Options Team areresponsible for preventing and relieving homelessness under the Homelessness Reduction Action 2017 and as well as managing the Council’s Housing Register.
Reporting to the Housing Options Manager you will carry out assessments of homeless applications to determine what statutory duties are owed to customers. You will then help customers either retain their existing accommodation or help them secure alternative accommodation.
You will have your own caseload and participate in the team’s duty rota.
The role requires excellent communication and negotiation skills with a wide range of people and organisations.
Your experience of working in a challenging front line homelessness or similar service will help you to remain calm under pressure while delivering the highest levels of customer service.
You will have a good understanding of the homelessness legislation and state benefits as well as having excellent IT skills and the ability to keep accurate records and meet deadlines.&nbsp;

As the Lifestyle Manager, you'll play a pivotal role developing our Lifestyle and Outdoor Recreation.
Your responsibilities will include:
<ul>
<li>Event planning - research, develop and facilitate new appropriate events and initiatives to enhance the cultural offer of the district in compliance with Arun District Council’s Events Policy framework, as well as creating new commercial opportunities for Tivoli.</li>
<li>Providing leadership, guidance, and support to your team, ensuring they have the tools and resources needed to succeed</li>
<li>Creating and delivery of a clear marketing strategy to increase usage of current Lifestyle sites, along with promotion of new opportunities.</li>
<li>Communicating in timely manner with clients and key stakeholders’ groups.</li>
<li>Regular organised ‘Town Hall’ events with user groups</li>
<li>Budget Management, including proposing budgets reporting to senior Leadership team</li>
<li>Monitoring stock levels and ordering as necessary.</li>
<li>Promoting a culture of diversity, equity, and inclusion within the team and fostering a positive work environment for all team members</li>
<li>Reporting usage figures to Arun District council when required</li>

What we're looking for:
<ul>
<li>Creative, motivated with a strong commercial understanding</li>
<li>Level 3/4 qualification in a Sports/Leisure/Events/Project Management related subject</li>
<li>IOSH or NEBOSH GC or willingness to obtain</li>
<li>Membership of an appropriate professional body e.g. CIMPSA</li>
<li>Strong communication and interpersonal skills, with the ability to effectively interact with team members, and external stakeholders</li>
<li>Developing and activating marketing and communications plans including promotions.</li>

The Mayor of London, Sadiq Khan, is reappointing his cohort of Mayor’s Design Advocates.
The Mayor of London is seeking independent built environment experts to join the Mayor’s Design Advocates (MDAs) cohort. MDAs play a crucial role in supporting the&nbsp;<a href="https://www.london.gov.uk/programmes-strategies/shaping-local-places/advice-and-guidance/about-good-growth-design" target="_blank">Good Growth by Design (GGbD) programme</a>, influencing Mayoral decisions, steering policies and advocating for high-quality, sustainable development across London.
Good Growth is the Mayor’s guiding principle for the built environment and sets the direction of the London Plan and all related Mayoral decisions, investments, strategies and policies. Good Growth acknowledges that Londoners will not accept growth at any cost. Growth of our built environment must be socially and economically inclusive, physically accessible and environmentally sustainable.
The programme draws together and promotes efforts being made across the Greater London Authority Group to support quality and inclusion in the built environment. With the support of the Mayor’s Design Advocates, it has had major successes: setting standards for excellence, extending the practice of design review, and supporting cross-industry action to make the built environment sector more diverse and representative. This programme is calling upon the architecture, design, planning, built environment and sustainability sectors to support Good Growth in London.

Eligibility:

Term of Appointment:

About the Role: 
Active Wirral are seeking 3 x Active Places Managers to support the Active Wirral management team with the ongoing service transformation. The modernisation of the management and support workforce within the Active Wirral service follows recommendations from a leading UK leisure consultancy.
The modernisation focuses on strengthening the operational management, facility and sports management and operational support workforce within the service. The service seeks to optimise efficiencies, whilst making meaningful saving towards the councils Medium Term Financial Plan.
The role will support and embed the principles of place-based working, including operational delivery that aligns with local priorities across Wirral’s communities, ensuring leisure facilities are active contributors to tackling inactivity, health inequalities and social isolation.
Active Wirral is responsible for the management and operations of 10 sports and leisure facilities across the Borough.
About the candidate: 
You will lead a team of Team Leaders and Duty Managers to deliver high quality sports and physical activities across the indoor and outdoor facilities and ensure the provision of safe, quality and customer focused service, delivering outcomes for customers through optimising value and best practice.
