Job Introduction

We are looking for a Reablement Worker to join our Short-Term Support Service in the West area, Dene Park House, Hexham on part time basis, 30 hours. There is a permanent post available and a fixed term post for 12 months.

The successful candidate will be part of a team providing short-term limited interventions to promote people's independence. The role typically involves visiting people in their own homes and supporting them to regain their ability to carry out a range of day-to-day tasks.

You will work with people to set their personal goals and help them to achieve them. This could include helping them with personal care and meal preparation, improving their mobility within their home and helping them to get out into their community.

Your focus will be on helping people to regain their ability to live their best life, and the feeling of job satisfaction is second to none. Full training and supervision are provided and there are opportunities to develop a career in care.

Some staff have moved to management positions in care and others have developed careers in allied areas such as undertaking social work qualifications or moving into commissioning and quality assurance roles.

If you have a caring nature, enjoy working with people, and would like to make a difference to people’s lives, we would love to hear from you!

About us:

Our Short Term Support Service provides reablement and rehabilitation to adults living in their own homes to help them regain their independence after an illness, hospital discharge, or to avoid unnecessary hospital admissions. We also provide a bridging service for people who are waiting for a long-term package of care. The service includes reablement and therapy staff working together to promote independence in daily living tasks, including the provision of equipment and rehabilitation programmes.

The service runs from 7am - 10pm, 7 days a week. You will be expected to work on a rota that includes AM and PM shifts.

Our service is rated Outstanding by the Care Quality Commission.

The main responsibilities include promoting independence with:

  • Personal care including showering and dressing
  • Medication
  • Meal preparation
  • Improving mobility within the home
  • Shopping
  • Laundry
  • Support to get out and about in the community

If this post appeals to you and you have caring qualities and would like a job that makes a difference, within a team known for its outstanding service!
