Our Integrated Discharge service in the West of Hertfordshire supports an integrated approach to managing the care of people who require support through the admission and discharge pathway, so the role will involve working alongside Occupational Therapists, Community Care Officers, health colleagues and care providers. You will have the ability to undertake innovative assessments and care planning with individuals, carers and their personal support networks.
We promote personalisation focusing on the wellbeing of the individual. Through care planning we strive for increased independence, choice and control by enabling people to direct their own support. You will gain extensive experience in the field of adult social care, safeguarding practice and multi-disciplinary working.
Out of County Hospital Integrated Discharge Team
Herts Valley
37 hours per week | Permanent
The Integrated Discharge Team within the Out of County Service provides a 7-day hospital discharge service. This is a full-time role with a requirement to work 1 in every 8 weekends (both Saturday and Sunday).This is a fast-paced service focused on supporting individuals to be discharged from hospital promptly and safely, using a person-centred approach. The majority of work is undertaken through the Discharge to Assess model. The standard working hours are 9:00am – 5:00pm. The role is primarily based at Luton Hospital; however, you must be able to travel to other out-of-borough hospitals where Herts Valley residents are admitted.
About the role
You'll ensure safe and timely discharges from hospital by:
- Assessing the needs of patients and carers
- Arranging appropriate care packages
- Liaising with professionals and agencies
- Supporting person-centred discharge planning
We offer a 7 day service and currently operate a 1-weekend-in-8 rota across the Hospital Team
About you
Essential:
• Social Work degree
• Registration with Social Work England
• Driving licence
