The service is registered with the Care Quality Commission and currently rated as Good. You will be involved in the recruitment of new carers, complete their assessment, training and approval. Once approved you will provide the ongoing support and monitoring needed to maintain the standards required and achieve the positive outcomes needed. The service provides short- and long-term placements within family homes for people who have care and support needs, taking a highly individual approach to care provision. Key responsibilities:

<li>To be a named representative and key link between Adult Social Care and Health. To liaise directly with GP surgeries about case work.</li>
<li>To have active contact with PCN’s, local GP surgeries, Community Mental Health Workers, Social Prescribers and voluntary services. To be physically present, building relationships and networking to promote better communication and joint working relationships.</li>
<li>To contribute towards the development of Multi-Disciplinary Team meetings within WBC, with a view to increased frequency and urgent responses. The role will also require chairing MDT meetings and acting as the Adult Social Care representative.</li>

We are looking for a dedicated and experienced Senior Social Worker, registered with Social Work England, to join our friendly, dynamic, and supportive multi-disciplinary team, where every day brings new challenges and opportunities.
You will work closely with both social care and health professionals, focusing on discharge planning for inpatients at our local Mental Health hospital.
You will play a key role in delivering timely assessments and implementing effective safeguarding plans, with the support of experienced senior staff and managers to guide you.

The Bracknell Adult Community Mental Health team is a dedicated team providing vital support to adults experiencing significant mental illness, as well as their carers. We are committed to safeguarding vulnerable adults, actively identifying and responding to risks to ensure their safety and wellbeing.
We offer a range of interventions, including medication management and psychological therapies, all while coordinating comprehensive, person-centred care within the community.
The team also fulfils statutory responsibilities under the Mental Health Act 1983, the Mental Capacity Act 2005, and the Care Act 2014.

<li>To undertake strengths-based and person-centred assessments to meet individual needs to be safely discharged from hospital.</li>
<li>To work collaboratively with professionals across the NHS and Social Care to holistically meet individual and carer needs.</li>
<li>To complete safeguarding enquiries, mental capacity assessments and best interests decisions.</li>
<li>To ensure that critical analysis and evidence-based practice is central to all decision making.</li>
<li>To ensure that all policies and procedures of the department are adhered to.</li>
