Our Team:

This is one of three roles that has recently been reshaped through a service restructure to strengthen our health improvement offering for older people. Reporting to the Health Improvement Specialist, you'll join a collaborative team committed to helping older adults improve their health, wellbeing and quality of life while reducing loneliness and supporting people to live independently for longer.

Working across communities, health and social care and care homes the team delivers practical, person-centred support that empowers people to access local opportunities, build confidence, increase physical activity and stay connected. We also work across the wider system to promote age-friendly approaches, strengthen partnerships and advocate for the issues that matter to older people.

Hours: 37 hour per week flexible working

Your Contributions:


You'll manage your own caseload, delivering the 12-week Stronger for Longer programme by completing holistic health and wellbeing assessments, developing personalised action plans, arranging access to appropriate community services and activities, and providing a handholding service where required to help people overcome barriers and build lasting confidence.

Working autonomously while supported by the Health Improvement Specialist, you'll coordinate referrals, undertake risk assessments, monitor progress and maintain accurate records to ensure positive outcomes for older adults.

You'll recruit and coordinate volunteers, organise community events, develop local service directories, identify gaps in provision and build strong relationships with health and social care partners, care homes and the VCFSE sector to create an age-friendly community.

You'll also play a key role in prevention by delivering training and workshops for frontline staff, including colleagues from care homes and social care, on preventative health improvement approaches. Using evidence-based public health principles, including Making Every Contact Count (MECC), you'll promote healthy ageing, increase awareness of local support and represent the service at meetings and community events to encourage referrals and collaborative working.

As an advocate for older people, you'll work with individuals, communities and third sector organisations to influence the wider system, ensuring the voices and experiences of older adults shape local services and opportunities.

You'll contribute to service development, performance monitoring and continuous improvement while maintaining the highest standards of safeguarding, health and safety, equality and customer care.

Essential - You will be trained to NVQ Level III in a related discipline.

As this role involves regular travel across the borough and sometimes further afield, a driving license and access to a vehicle are essential requirements. Where appropriate, reasonable adjustments will be made in accordance with the provisions of the Equality Act.
