You will work closely with colleagues across Adult Social Care, housing partners, contractors and Housing Associations, contributing to creative, person‑centred solutions that make a real and lasting difference to residents’ independence and wellbeing.

About you...
You will be:
- A HCPC‑registered Occupational Therapist with a recognised qualification
- Experienced in major housing adaptations and Disabled Facilities Grant work
- Confident working collaboratively with colleagues, service users and partner organisations
- Able to assess complex housing needs and identify practical, proportionate solutions
- Interested in developing your specialist knowledge and contributing to service improvement
- Experienced in assessing and matching individuals to alternative or void properties when adaptations are not suitable

About the team...
The Disabled Facilities Grant team is a small, busy and supportive service that works closely with Adult Social Care, housing partners, contractors, and Housing Associations.
We deliver person-centred adaptations that meet long-term needs and ensure the best outcomes for our clients.
You will be joining a team that values professional trust, reflective practice and mutual support.
Key responsibilities...
- Undertake person‑centred assessments to identify housing adaptations that promote safety, independence and long‑term wellbeing
- Work collaboratively with service users, colleagues and partners to develop and progress DFG solutions
- Contribute specialist OT knowledge to multidisciplinary meetings, case discussions and service development
- Produce clear, evidence‑based reports and recommendations to support DFG applications and adaptation specifications
- Maintain accurate electronic records in line with professional and organisational standards
- Engage fully in supervision, appraisal and ongoing professional development
- Practice in accordance with council policies and the HCPC Code of Conduct and Standards of Proficiency
