We work with residents of North Somerset to support them on their journey from hospital to home, in line with the Care Act 2014 and with regard to the Mental Capacity Act 2005. We follow the Hospital Discharge and Community Support guidance.We are a friendly and supportive team made up of Senior Social Workers, Social Workers and Adult Social Care Workers. We undertake Care Act assessments, and mental capacity and best interest assessments.
