Exciting things are happening in Adult Social Care on the Isle of Wight. Our service reflects our community-focused, strengths-based approach and brings key teams closer together.
As a result, we are looking for two Social Workers to join our Wellbeing & Access Hub, supporting people at their first point of contact. This is a dynamic role working alongside Outreach and Occupational Therapy colleagues to reduce the need for long-term care. You’ll help people regain independence through reablement, provide clear information and advice, and signpost to a wide range of third-sector services.
As part of the front door to Adult Social Care, you’ll handle telephone referrals, carry out visits, and complete Care Act assessments. You’ll deliver timely, strengths-based assessments and support plans that promote independence, choice and autonomy.
You’ll also play a key role in safeguarding, follow departmental recording standards, and contribute to multi-agency discharge planning to support safe, timely transitions. Supporting carers is central to the role too, including offering Carers Assessments in line with the Care Act 2014

Salary range
