Our Team:

The Transitions Team is made up of Social Workers and Community Care Workers who support young people aged 14 to 25 with disabilities. Their focus is to assist in the transition from Children’s Services to Adult Social Care. In line with the duties and responsibilities set out in the Care Act 2014, the team collaborates with a range of services; statutory, voluntary, and private sector, to build strong partnerships and deliver improved outcomes for the individuals they support. A key priority is ensuring that the Preparing for Adulthood agenda is fully embedded in their practice.

Hours: 22.5 hours per week

Your Contributions:

As an experienced Social Worker within the Transitions Team, you will support the operational implementation of Strengths and Rights Based approaches within Adult Social Care. You will also support in the development of the teams approach, focusing on Continued Professional Development

This role offers an excellent opportunity to develop and enhance your skills in Social Work, while actively contributing to the development of effective partnership working with health colleagues, aimed at improving the experience and outcomes of adults accessing social care services.

Applicants must have demonstrable experience in undertaking Section 42 Safeguarding Enquiries, court work, and risk assessments. You should also have a proven ability to work collaboratively with a wide range of services, stakeholders, and professionals. Experience in Children’s Social Care is desirable, particularly in relation to supporting smooth transitions. An understanding of the Preparing for Adulthood agenda is essential to this role.

Please note that this is an essential car user post. You must hold a full, valid driving licence and have access to a vehicle for work purposes.
