We are delighted to offer a fixed-term Social Worker secondment for six months within our Community Mental Health Team for Older Adults.

About you

As a Social Worker, you are a compassionate and motivated professional with a strong commitment to person-centred care. You value collaboration and bring a proactive approach to supporting individuals with mental health needs and memory difficulties.

Confident in working with partners across health, social care and the voluntary sector, you are skilled at building relationships that promote independence and wellbeing. You are organised, adaptable and comfortable managing complex situations, including safeguarding responsibilities. Most importantly, you are passionate about empowering people to live as independently as possible while ensuring they feel safe, heard and supported.

About the team

Based within the Bracknell Community Mental Health Team for Older Adults, you will join a team known for its outstanding clinical expertise and supportive culture. Colleagues are dedicated, hardworking and always willing to help one another, creating an inclusive and welcoming environment. Managers are approachable and actively encourage every voice to be heard.

The team shares a clear ethos centred on empowering individuals to maintain and grow their independence. With strong administrative support and excellent partnerships across agencies, we work collaboratively to provide high-quality care and meaningful outcomes for our most vulnerable residents.

Key responsibilities

  • Provide advice, information, support and care to people with diagnosed memory problems and those with mental health needs requiring social care.
  • Help coordinate available resources to identify individuals requiring longer-term support and deliver short-term interventions using strengths-based assessments and community assets.
  • Support individuals with social care needs to assess their own requirements and develop person-centred support plans, working collaboratively with carers, family and friends, health professionals and the voluntary sector.
  • Take responsibility for assessing safeguarding referrals, including carrying out investigations, completing risk assessments, chairing meetings (where necessary), and ensuring that action plans effectively keep people safe.
  • Establish good clinical and management practices, ensuring high standards of care through the use of community and other available resources.
