Barnsley Adult Social Care is seeking a qualified Social Worker to join the Review Team, undertaking Care Act assessments and reviews across the borough to ensure people receive the right support at the right time, promoting independence and safe, sustainable outcomes. The role involves working with adults aged 18+, including older people, those with physical disabilities, and individuals experiencing complex vulnerabilities such as self-neglect and exploitation, across community, care home, and hospital discharge pathways. You will apply key legislation including the Care Act 2014 and Mental Capacity Act 2005, undertake safeguarding work in line with Making Safeguarding Personal, and work collaboratively within multi-disciplinary teams, including health and reablement services, to support timely discharges and strengths-based practice.
