    Social Workers - Adults Intermediate Care Team

    We are recruiting to a number of Social Worker vacancies to work in our Adults Intermediate Care Team and welcome applications from Newly Qualified through to Senior Social Workers.

    (Newly Qualified, Social Worker, Senior Social Worker) - depending on experience:

    Salary:

    NQSW Band 8, £36,363 - £39,151 per annum

    Social Worker Band 9, £39,151 - £42,838 per annum

    Senior Social Worker Band 10, £42,838 to £47,181 per annum

    Pay band and salary will be dependent on experience. Social Workers who have not completed the Assisted and Supported Year of Employment will be supported to do so and would be paid on Band 8 and moving to Band 9 on completion. Social Workers with experience are paid on Band 9 are expected to supervise unregistered staff. Senior Social Workers are expected to supervise Social Workers, take on the most complex cases and would be paid on Band 10.

    The Service:

    There have been exciting changes within Portsmouth Adults Intermediate Care Team, which went live on 1 April 2024 and has brought together a number of teams to provide a single approach to supporting people who are moving through admission avoidance, the acute hospital, inpatient settings, and rehabilitation and reablement services. This is an exciting time to join us and help us on our journey to improving outcomes for people following a crisis or decline in health.

    The post is based within the Portsmouth Adults Intermediate Care Team (AICT) supporting our integrated services including Portsmouth Community Rehab Service, The Transfer of Care Hub, The acute hospital (Queen Alexandra Hospital) and Inpatient units.

    We are looking to recruit social workers to work across all of these areas, so would be the ideal opportunity for someone seeking a variety within their work. One day you may be working with a person in hospital and supporting them with their discharge planning, ensuring a home first approach at each stage, the next day, visiting a person in their own home, supporting them back to independence through the application of the strengths -based approach.

    Your New Role:

    You will join a team of Social Workers and Social Care Associate Practitioners based within the Portsmouth Adults Intermediate Care Team. Your role will involve assessing the needs of adults and their Carers, to identify creative ways to meet their assessed outcomes under the Care Act 2014.

    The role includes outcome led interventions, using astrength-based approach to support people to live the lives they chose. You will work collaboratively with others, maintaining a strength based and aspirational approach to achieve the outcomes the person wants.

    Duties include assessing clients’ needs and agreeing a person-centred support plan, completing reviews of assessments and support plans, supporting people with their health, mental health, and social care needs, and assessing mental capacity when required. You will work independently, manage your own caseload, and have opportunities tosupport unqualified staff and pursue your own personal learning and development.

    What you'll need to succeed:

    You will have a qualification in social work and be registered with Social Work England. You will have knowledge of safeguarding adults, and a thorough understanding of adult social care legislation. You can communicate with a diverse range of people, in sometimes challenging circumstances. You manage risk and maintain records effectively and confidentially and can collaborate effectively whilst being an advocate for individuals and carers. You are professionally curious and have a positive approach to motivating others.
