Ready to be part of a service that is delivering real change for our communities? We are looking for a passionate and values, driven Social Worker to join our Adult Social Care team, working at the heart of our strategy for change to provide the right support, at the right time, in the right way.

You’ll help shape a system that priorities prevention, strengths and community based solutions to improve outcomes for people.

You will lead on assessments, care planning and reviews using a strengths based, person centred approach, working alongside partners, families and communities to co-produce solutions that build on what matters to people. You’ll support independence, manage risk proportionately and contribute to integrated, outcome-focused practice that reduces reliance on long-term services while ensuring people are safe and supported.

We are looking for a qualified Social Worker with strong professional curiosity, sound knowledge of legislation and a commitment to doing things differently.
