An opportunity has arisen for a twelve month fixed term post to lead the operational design and delivery of the local Neighbourhood Health agenda within Knowsley.

Under the neighbourhood health framework partners will work collectively and with the local population to shift care into communities, develop local neighbourhood health plans and improve local health outcomes. Using population health approaches, prevention will be a focus as well as joining up action around wider determinants to target deeply ingrained inequalities in health

The postholder will work closely and effectively with internal and external partners including Senior Managers across the Council, Cheshire and Merseyside Integrated Care Board, Acute Hospital Trusts, Community Health Providers, Primary Care Networks, Elected Members, Service Users, Carers, Provider Organisations, and the Voluntary Community Faith and Social Enterprise Sector, to ensure that stakeholder views are understood and are reflected in emerging Neighbourhood Health Plans. This will include ensuring multi-agency neighbourhood teams are centred around neighbourhood hubs and work closely with local residents to improve health and wellbeing outcomes.

The postholder will sit within the Public Health Team, providing a focus on intelligence and evidenced based practice, led by insight and co-design with communities with inequalities at its core.
