We are seeking a permanent Service Manager to lead and support discharge and patient flow across a diverse, city-wide footprint. This is an exciting opportunity for an experienced professional to take on a pivotal leadership role focused on coordinating and streamlining patient discharge processes.
As part of our Integrated Discharge Team, you will play a key role in delivering timely interventions for adults who present with care and support needs. You will champion a strengths-based approach in completing Care Act assessments, ensuring compliance with relevant legal frameworks, and leading on Section 42 safeguarding enquiries. Your work will drive inclusive, collaborative, and innovative support planning creating sustainable solutions that make a real difference.
The role:
- The Service Manager will provide leadership and guidance to the team, including supervision of Team Managers based across multiple hospital sites
- Strengthen partnerships with health services and other agencies through collaborative, strengths-based practice to enable efficient and effective patient flow
- The Service Manager will ensure robust discharge planning and coordination that promotes independence and positive outcomes for individuals
- Support the leading of safeguarding enquiries and uphold statutory responsibilities under the Care Act.
- The Service Manager will drive continuous improvement and innovation within discharge processes to enhance service delivery

Salary range
- £63,819 - £70,608 per year
