About the team

Transforming Care is a national programme implemented by NHS England to transform the care and support for children, young people and adults with a learning disability and/or autism who may be in hospital/at risk of hospital admission, or possibly in contact with the Criminal Justice System.

About the role

You will work at the forefront of best practice to deliver personalised support to vulnerable adults with the most complex care needs. You will have the skills necessary to develop discharge pathways from psychiatric hospitals and in prevention work to avoid inappropriate admission to hospital. Your work will be adult focused but within a team that offers an All Age approach across the fields of learning disability and autism. You will work alongside wider professional agencies including advocacy, health and local and national commissioner groups to effect change.

Essential to the role is the ability to see the potential in people and to take a creative and proactive approach to risk and problem solving despite being challenged to the contrary. You will understand the complex and often conflicting matters of enabling ordinary lives whilst protecting people including the public from harm, and the role legal frameworks play. You will be able to progress and keep people at the centre in ‘stuck’ cultures and systems.

About you

Essential:

  • Qualified and registered Social Worker/Occupational Therapist /Community Nurse adhering to national professional standards.
  • Experience and knowledge in learning disabilities, autism, mental health, and relevant legislation.
  • Experience in assessing capacity and best interest decision making and in the understanding of Deprivation of Liberty Safeguards.
  • Experience in working and communicating effectively across MDTs, family, advocates, and wider stakeholders such as commissioners and health/social care leads.
  • Excellent communication and organisational skills.
  • You will be expected to work autonomously, able to Smart Work and visit out of county placements.
  • You will have excellent skills in assessment and report writing.
  • Experience of complex case management and working with individuals at increased risk of harm.
  • Driving licence and access to a vehicle – ability to travel within and outside of the county.

Desirable:

  • Ideally you will have experience of working with people placed in inpatient settings; you may have experience of working with people in a forensic setting.
  • Ideally you will have experience of tribunal and court of protection work including submitting evidence and court attendance.
