    The Discharge, Assessment and Response Team (DART) are excited to offer a fantastic opportunity to an enthusiastic, professionally curious, and motivated Senior Social Worker to join our friendly, supportive, and high performing Hospital Social Work Team. This post is to join the community arm of the team, assessing those who have recently experienced a hospital admission and have been discharged on the Discharge to Assess (D2A) pathway either to their home or into a rehabilitation unit. You will work closely with health professionals/therapists to coordinate a safe and timely discharge from the hospital discharge pathways.

    Position Available:

    • 1 Full-Time permanent Senior Social Worker position.

    About the role

    About the Role:

    • We are looking for someone with strong assessment, communication, and leadership skills, with relevant experience completing and supporting complex short-term casework.
    • You must have high professional standards, and be keen to continue your professional development and share your knowledge and skills.
    • To have a commitment to supervision and to support and supervise our team of assessment workers (Social Workers and Social Work Assistants) who support patients and their families through the often complex hospital discharge pathway and care system, enabling their safe discharge from the D2A pathway.
    • To promote and support excellent Social Work practice providing guidance to the team.
    • To work in partnership and contribute to a multiagency approach with our system partners and health colleagues.
    • You should have a comprehensive knowledge of relevant legislation, including the Care Act 2014 and Mental Capacity Act 2005, in addition to having excellent knowledge around Safeguarding vulnerable adults and the management of risk and demonstrate confidence in and supporting others in undertaking Section 42 safeguarding enquiries.
    • Collaborate with our team, our health colleagues, families, and carers, adhering to statutory legislation and embedding strengths and relationship-based practice in assessments.
    • Participating in leadership and decision-making initiatives, contributing to the ongoing development of DART - Hospital Social Work service, quality improvement and development.
    • Engaging in ongoing professional development to stay current with Social Work best practices and changes in the field of Social Work and changes in Brighton and Hove Local Authority.
    • Lead and mentor our Social Workers and Social Work Assistants, sharing your expertise and fostering a supportive learning environment and culture, maximising our team’s wellbeing and potential.

    Our Team's Focus:

    • Delivering high-quality, strength based and person-centred Social Work with clients and their families to enable safe discharges from the D2A pathway. This includes undertaking Care Act Assessments and Mental Capacity Assessments and coordinating often complex discharges with other involved professionals.
    • Undertaking Safeguarding and Section 42 Enquiries for vulnerable adults.
    • Professionalism and a commitment to ongoing professional development are the keystones of our practice and very much encouraged.
    • Working collaboratively & in partnership with our health colleagues with a focus on best outcomes for the patient.

    Our Work Environment:

    • We pride ourselves on being a friendly and supportive team, thriving in a busy and sometimes challenging system. We work with a hybrid model and our office base is Hove Town Hall. We are surrounded by local shops and cafés and just a short stroll to the sea. There is paid parking around the Town Hall. Hove train station is a 10/15 walk from Hove Town Hall.
    • We understand the benefits of hybrid flexible working but also very much value time in the office together given the nature of our work and appreciate the support we can offer one another.

    Our Team

    • Our Social Work team consists of 1 Operations Manager, 2 Senior Social Workers (including this post), and 12 assessment workers.

    Additional information

    Your starting salary will be pro rata if the above position is less than 37 hours or term-time only.
