Job Summary:

An opportunity has arisen for Social Workers and Senior Practitioners who are seeking to further develop their professional practice or broaden their experience within an acute hospital setting.

We are inviting Expressions of Interest for two permanent Senior Social Worker roles within the Hospital Team. These posts are suitable for experienced practitioners who demonstrate a sound understanding of the hospital discharge pathway or who can evidence the ability to learn quickly and operate effectively within a fast‑paced, multi‑disciplinary environment.

Role Purpose

The Senior Social Worker will play a key role in supporting the effective and timely discharge of adults from hospital by providing operational oversight, professional leadership, and practice supervision. The postholder will be responsible for managing patient flow across allocated hospital sites and ensuring that statutory duties are met in line with legislation, policy, and best practice.

Key Responsibilities

  • Manage flow within allocated hospitals, including allocating cases and overseeing workload distribution.
  • Provide formal supervision, mentoring, and professional guidance to Social Workers, supporting development and improving practice standards.
  • Undertake complex strength-based assessments and make sound analytical judgements in collaboration with health partners and external agencies.
  • Ensure high‑quality, timely, and accurate case recording, with strong working knowledge of Mosaic.
  • Provide information, advice, and professional decision‑making support on complex cases.
  • Support consistent, lawful, and proportionate practice in line with legislation and local procedures.
  • Work collaboratively with health colleagues, partner agencies, and community resources to achieve positive outcomes for adults.
  • Working collaboratively with management to support service flow while effectively balancing competing managerial responsibilities.
  • Contribute to service development, quality assurance, and continuous improvement.
  • Total adherence to Ealing’s core values.

This role includes formal supervisory responsibilities and contributes to both operational delivery and practice development across the team.

About the Team

The Hospital Team is a large, established service comprising approximately 20 practitioners, bringing a wide range of skills, experiences, and professional backgrounds. The successful candidates will be one of three Senior Social Workers forming part of the management structure, working collaboratively to deliver a high‑quality service that:

  • Embeds person‑centred practice.
  • Promotes a supportive, inclusive, and reflective team culture.
  • Balances operational demands with effective resource management.
  • Supports the economic sustainability of services in Ealing.

There is an expectation that Senior Social Workers maintain a visible presence within their allocated hospital sites, both on‑site and virtually, to support staff, strengthen partnerships, and promote effective joint working.

Person Specification

Applicants should be able to demonstrate:

  • A commitment to delivering high‑quality services to adults in line with legislative and policy frameworks.
  • Strong knowledge of hospital discharge processes, or the ability to develop this quickly.
  • Experience of supervising staff and supporting professional development.
  • The ability to work collaboratively across organisational boundaries.
  • A proactive, forward‑thinking approach to service delivery and problem‑solving.
  • The capability to positively influence team culture and practice standards.
