Our Team:

This role is part of Halton Health Improvement Team within Public Health and is accountable to the Health Improvement Principal Manager. The post-holder will be responsible for managing the well-established ‘Live Well’ team who deliver on programmes to promote healthy lifestyles and reduce the risk of long-term health conditions for people who live and work in Halton.

You will provide strategic leadership and expertise on adult healthy lifestyles, working alongside other Practice Managers within the wider Health Improvement Team.

You will be based at Runcorn Town Hall, with the option of flexible hybrid working. This post is fixed term expected until 31st March 2028 to cover a secondment.

Hours: 37 per week

Your Contributions:

The Practice Manager for Adult Healthy Lifestyles provides leadership across Halton’s Public Health prevention services for adults, and is responsible for leading and developing evidence‑based programmes aligned with Public Health priorities, including NHS Health Checks, Exercise on Referral and adult weight management.

The postholder drives service improvement through the use of local population needs assessments, data, intelligence and best practice.

Working autonomously, they lead and motivate a multidisciplinary team and build strong partnerships across primary and secondary care, social care and community sectors.

The core focus of this role is improving health outcomes for adults in Halton and actively reducing health inequalities through targeted, integrated and person‑centred prevention approaches.
