Are you passionate about working with people to make key lifestyle changes to improve their health and wellbeing?

We are looking for a highly motivated health professional to join our One You Health Improvement Service at Dartford Borough Council. We are part of an innovative Health Improvement partnership in the Dartford, Gravesham and Swanley area and are commissioned by KCC to deliver the One You Kent health service in Dartford.

Working as part of a dedicated and supportive team, you will carry out one to one health and wellbeing assessments, supporting people to lose weight, move more, stop smoking, drink less and improve diet and mental wellbeing. You will also support delivery of our successful weight management programmes and other preventative health initiatives

Participation in community and other One You promotional events will form key areas of work will include travel to venues and locations across the borough.

You will work collaboratively with colleagues within the Council, voluntary, community and health sectors to ensure that the customer is at the centre of a holistic approach to their health needs.

You will be highly motivated and able to build good rapport with individuals within the community using excellent communication skills. You will have an up-to-date knowledge of relevant health and wellbeing interventions and a passion for developing this knowledge base.
