We are recruiting for a Mental Health Social worker to join our Bow & Poplar Neighbourhood Mental Health Team.
If you are passionate about social work and promoting recovery mental health and rehabilitation you will have to be passionate about delivering excellent social work in promoting strength based approach, committed to achieving positive outcomes, and bring years of experience/qualification requirements, we would love to hear from you.
In this role, you will:

Complete person centred assessment under the Care Act 2014

Complete Safeguarding Enquiry in line Care Act 2014

Undertaking Care Coordination role to work in collaboration with the client and carers
-
To work as part of Multi-diciplinary team to promote recovery.
About You
We are looking for someone who can demonstrate:
