Scale 4 (£29,064 - £32,597) per annum
37 hours per week – Part time/Job Share to be considered for the right candidate
12 months fixed term with possibility of extension
This role is an Essential Car User post and will receive payments based on the Council’s Essential Car User policy

The Housing Enablement Team (HET) are looking for an experienced and knowledgeable individual to provide housing related support to patients in hospital in order to assist them to be safely discharged home or into new accommodation.
Your role will be to provide short-term case management and organise support services for individuals and families to enable discharge. You will support patients to secure and move into independent accommodation or to return home, assisting with the practicalities of moving. You will supply food parcels and heaters, assist with utilities and provide practical benefit/income support. You will also be required to visit properties to assess their state for discharge and arrange and meet with contractors in order to facilitate cleaning or clearances of cluttered properties.
You will have previous experience of working within a support, care or housing setting dealing with vulnerable people with a range of support needs, undertaking basic assessments, support planning, carrying out action plans and reporting outcomes.
You will also have an understanding of issues facing vulnerable people, including those with mental health issues, with housing related support needs including dependency needs, tenancy sustainment, homelessness and general life skills.
This post benefits from hybrid working. You will be based from home or from office space located at the Leicester Royal Infirmary (LRI). The post will cover the three UHL hospitals (LRI, Glenfield & Leicester General), all Community Hospitals in Leicester, Leicestershire & Rutland, the Bradgate Mental Health Unit, the Mental Health Rehabilitation Units and the Mental Health services for Older Persons (MHSOP) wards.
This role will require an Enhanced Disclosure and Barring Service check (known as a DBS check). All offers of employment will be made conditional upon receipt of the satisfactory DBS check. You must have a vehicle, full driving licence and business insurance to be able to travel around Leicester City, Leicestershire County and Rutland in order to undertake duties.
