We have an exciting opportunity for an Autism Transition Outreach Worker, on a full-time, term-time only basis.
About you...
You will support autistic children transitioning from mainstream primary to secondary schools within Bracknell Forest. Working with a select cohort, you will build strong, trusting relationships with children, their parents/carers and schools to ensure a smooth and successful transition. You will hold a case load of pupils, across KS2 and KS3, working in partnership with primary and secondary educators throughout.
We are seeking someone with a HLTA Level 4 qualification or relevant qualification in the field of autism. You will be a confident communicator, able to engage with neurodiverse children and work effectively with families and professionals to meet individual needs.
About the team...
You will be working closely with the Autism service who are committed to supporting neurodiverse children within mainstream settings across Bracknell Forest. The role is varied and stimulating with opportunities to work in partnership with our schools whilst identifying and delivering the transitional support our students require. Whilst we collaborate effectively within a close supportive team, we also maintain the initiative and discipline to manage our roles independently.
Key responsibilities...
- To meet the transitionary needs of autistic children as they move from primary to secondary through the delivery of a seamless transitionary programme, aiming for children to be confident and thriving in their secondary setting
- To hold a caseload of children transitioning between primary and secondary settings, ensuring needs are appropriately captured and understood - reporting on the same to schools, parent/carers and the local authority
- To form positive working relationships with primary and secondary educators, to support autistic pupils' transition
- To promote inclusion and establish constructive and trusting relationships with pupils and parents/carers, providing advocacy for the children following their transition to their secondary setting
