
Adults Senior Social Worker - Frimley Park Hospital

This role has a starting salary of £47,884 per annum based on a 36 hour working week. This is a full-time position; part-time applications will be considered.

Frimley Park hospital is a 7-day service, however the Senior Social Work role in the D2A team is a Monday to Friday position but may entail weekend and Bank-holiday working should the needs of the service require this.
This opportunity would suit either a Senior Social Worker looking to expand their knowledge and skill-set, or equally for a Social Worker looking to progress their career and gain valuable experience and knowledge, all whilst supporting our residents to achieve positive outcomes.

About the Team
Frimley Park is a busy, acute hospital. The Hospital Social Work Team is a dynamic and fast-paced team comprising of Social Workers, Social Care Assistants, and Administrators that work with multiple professionals to ensure the safe discharge and follow up care. About the Role
Working as part of the D2A team provides a fantastic range of residents to work with, both in their own home and in care home settings. We foster and promote positive, collaborative working relationships with internal and external partners, all with the aim of supporting our residents to live well and as independently as possible.
We offer regular supervision to all our staff every month, plus we provide informal support from the wider team. The value of occupational therapists is well recognised within our team, which enables us to fully utilise our skills and strengths to achieve positive outcomes, focussing on what is important to the individuals we work with.
You will have frequent direct contact with the public as part of the D2A function, visiting our residents and their families in their own home or in a care home placement. Your role will involve a variety of tasks including assessing and reviewing people's needs to promote their independence and enable them to continue living, or returning to their own home. You will create and maintain positive working relationships with internal and external partners to promote the well-being and safety of vulnerable residents whom you come into contact with.
You will work in partnership with individuals, their carers, professional staff in Surrey County Council, other agencies, and service providers in order to support vulnerable adults and their families to identify and arrange services that meet their needs and promote independence, choice and control.
You will have a caseload, working with individuals who need additional support or guidance in order to identify their needs and carrying out risk assessments. On a daily basis, you will be meeting with clients to understand their care and needs, how they can be supported and to provide the best outcomes for them. Your work will include but is not limited to:
- Care Act assessments
- Carers assessments
- Safeguarding work
- Partnership working closely with our community Health, charity and voluntary organisations.
- Mental capacity assessments

Shortlisting Criteria
To be considered for shortlisting for this position, your application will clearly evidence the following:
- Professional qualification in Social Work and current registration with Social Work England
- Experience of working within a social care, health or community focused multi-disciplinary setting
- Ability to complete person centred and proportionate Social Work assessments including Section 9 and 10 Care Act assessments as well as Mental Capacity Assessments and Best-interest decisions
- Familiarity with key legislation such as the Care Act and Mental Capacity Act and its application to practice in a social care setting.
- Knowledge of medical conditions and the likely impact on the individual, carer and family
- Substantial post-qualification professional experience and experience of providing professional supervision (support and training to further these skills will be provided)

Surrey has both urban and rural areas and locality based social care workers will be expected to have a valid driving licence to drive in the UK, access to a vehicle and be willing to travel across a wide geographical area.
