A new exciting opportunity has arisen within the Adults with Learning Disabilities and Autism Team for an Adult Practitioner to join us.
We work with those with an LD and/or Autism across Telford and whilst we are an adults team, our Transition workers are involved with those coming through that process from aged 14.
The ALD Team are passionate about supporting people to live fulfilling lives as valued members of their community. A key area of focus in enabling people to live independently and maximising their independence, as well as supporting carers to maintain their role. The Team also supports young people to prepare for adulthood and the multiagency working is vital in ensuring people have the right support that meets their individual needs and desired outcomes.

You will be expected to work in a person-centred way to enable adults who are Autistic and/or diagnosed with a Learning Disability, to reach their full potential, maximise their social inclusion and achieve positive outcomes.
We use strengths and asset-based approaches in our practice; how individuals can be supported by the strengths they have in themselves, their network of support from family, friends and their local communities. Where care and support are required, we work to ensure that this reflects the individual’s assessed needs and strengths while promoting their independence, choice and control.
Our Adult Practitioner work with the individual, their carers and families, to build a Support Plan, with a broad range of interventions that will provide a provision that is holistic, evidenced based and delivered within communities.

Salary range
- £31,537 - £33,699 per year
