This is a very exciting opportunity for an experienced, highly skilled and motivated practitioner to support Medway’s improvement work by leading and strengthening our Transition approaches for young people with Special Educational Needs and Disabilities (SEND) from Year 9 onwards (Key Stage 4) as they move towards Post‑16 education, training and adulthood.

We are seeking to recruit an outstanding and appropriately experienced practitioner with a strong track record of delivering person-centred support to young people with SEND, ideally within Key Stage 4, Post‑16, or transition-focused roles. Experience of working directly with young people and their families to plan for future pathways, employment options, independent living and wider Preparation for Adulthood outcomes is essential.

The successful candidate will manage a caseload of young people with complex SEND and will take the lead on co-ordinating, organising and chairing Annual Reviews, ensuring high-quality, timely updates to Education, Health and Care Plans (EHCPs). This includes responsibility for ensuring Year 9+ reviews effectively plan for transition and reflect clear, achievable next steps into Preparation for Adulthood.

This is a fantastic opportunity to join a child‑focused, multi‑disciplinary service working closely with secondary schools, colleges, training providers, health professionals, Early Help – Family Solutions, Social Care and a range of partner agencies.

We are looking for a highly organised, confident and assertive officer who is able to manage challenging conversations, support families through key points of change, and promote positive and ambitious outcomes for young people as they transition into adulthood.

Working under the direction of the Head of SEND, the postholder will also contribute to developing and embedding pathways, guidance and models of best practice that strengthen transition from secondary school into Post‑16 education, training or adult services.

The successful post holder will be subject to an enhanced DBS check.

Career Progression Frameworks (CPF) have been introduced for all roles at Medway Council. These frameworks provide a mechanism for employees to take ownership of their development within their post and be rewarded accordingly. Level A

SEND are seeking an EHC Coordinator for Pre-14 to join the team at level A. This is the entry point of the role, so you will need to be able to demonstrate the relevant knowledge, experience and skills to get you started, but you will be supported to develop in your role through induction, probation and a supportive Performance Appraisal and Career Conversation. All elements of level A listed within the person specification are essential. Through your induction you will be introduced to the Career Progression Framework (CPF), which will help you to identify your areas of strength and any development needs.
