    Your new role

    Join Portsmouth City Council's Adult Care and Support Community Social Work Team and make a real difference to the lives of adults with care and support needs and their carers.

    In this rewarding role, you will work with people with complex needs, carrying out strengths-based assessments, developing personalised support plans, and helping individuals achieve the outcomes that matter most to them.Working collaboratively with service users, carers and partner agencies, you will use professional judgement and evidence-based practice to identify the most effective solutions and support.

    You will manage a varied caseload autonomously, ensuring your practice is proportionate, legal, accountable and necessary, while maintaining high-quality records and safeguarding standards.Working as part of a multidisciplinary team, you will also supervise staff, contribute to service improvement initiatives, and support the ongoing development of excellent social work practice.

    We offer regular supervision, professional support, and opportunities for continuous professional development, helping you maintain and enhance your skills in line with Social Work England requirements.

    Who is the person summary:

    We are looking for a qualified Social Worker with experience of working with adults, ideally within a fieldwork or safeguarding environment.You will have a strong understanding of adult social care legislation, including the Care Act, Mental Health Act, Mental Capacity Act and Human Rights Act, alongside a commitment to safeguarding and promoting the welfare of adults and children.

    You will be skilled in assessment, risk management and case recording, with the ability to analyse complex information and make informed decisions.A creative, flexible and person-centred approach is essential, together with strong problem-solving and advocacy skills to support individuals and carers in achieving positive outcomes.

    Excellent communication and relationship-building skills are key, as you will work collaboratively with individuals, families, carers and partner agencies, often in challenging circumstances.You will be motivated by promoting choice, independence, inclusion and wellbeing, and capable of producing clear, evidence-based records of interventions and outcomes.

    Experience of supervising staff and working with substance misuse services would be an advantage.

    Local Government Reorganisation (LGR)

    As part of Local Government Reorganisation, current plans will see Portsmouth City Council combine with the district and borough councils of Fareham, Gosport and Havant and parts of East Hampshire and Winchester district councils to form a new, larger unitary authority for south east Hampshire.
