Roles are either ‘site based’ or ‘home based’.

Site based roles are mainly based at the workplace, with the option to work from home for up to two days a week, depending on service needs.

Home based roles are mainly based at home, with the expectation of attending the workplace up to one day a week.

The working arrangement for each role is set by the manager, based on service requirements.

Job Introduction

We’re recruiting a Educational Provision Practitioner to join our dedicated EHCP Provision Team at Milton Keynes City Council. The EHCP Provision Team delivers and manages provision for children and young people with education, health and care plans – the large majority of our work is statutory and delivers and manages education provision for the most vulnerable children and young people in Milton Keynes. By ensuring the team is at full capacity we will be able to ensure that there is no break or pause in this provision.

In this rewarding role, you’ll work directly with children and young people with special educational needs and disabilities, helping to shape education provision that meets their individual needs and supports positive outcomes. Your contribution will ensure children and young people receive consistent, high-quality support, particularly at key transition points or when education placements change.

Working collaboratively with families, education settings and partner agencies, you’ll help ensure that no child falls behind and that provision remains effective and responsive.


Site-Based role

This is a Site-Based role, and you’ll be expected to work from the designated workplace.

Role Responsibilities
  • Deliver high-quality support by attending and facilitating annual reviews for children and young people with education, health and care plans, ensuring all statutory requirements are met.
  • Lead on designing and implementing personalised education packages that support children and young people who are out of school or transitioning into new settings.
  • Communicate clearly with families, schools and partners to explain legislation, processes and outcomes in a way that encourages engagement and understanding.
  • Work collaboratively to support the wider SEND partnership, contributing to service improvement and positive local outcomes.
  • Ensure effective monitoring of progress by setting clear outcomes, tracking impact and maintaining accurate records for your caseload.
  • Take responsibility for working with a range of professionals and services to provide joined-up support that keeps safeguarding at the centre of all activity.
Ideal Candidate
  • You bring a relevant qualification such as NVQ level three in a childhood development field, or equivalent experience working with children and young people with special educational needs and disabilities.
  • Your ability to organise and prioritise ensures you can manage a varied caseload while maintaining attention to detail.
  • Experience in working with children and young people with SEND allows you to understand their needs and support positive outcomes.
  • A strong understanding of communication enables you to explain complex information clearly to a wide range of audiences.
  • Confidence when working with partners and families helps you manage challenging conversations in a calm and professional way.
  • Proven skills in collaboration allow you to work effectively with colleagues and external partners in a respectful and inclusive way.
  • An approach that demonstrates resilience ensures you can adapt to the evolving SEND landscape and respond to changing service needs.
  • Willingness to travel and a full driving licence support your ability to deliver services across Milton Keynes.

Full UK Driving Licence and access to a vehicle

To be successful in this role, you’ll need a full UK driving licence and access to a vehicle. If we offer you the role, we’ll ask you to provide evidence of your driving licence and vehicle insurance, including cover for business use. We’re unable to confirm an appointment until these documents are received.

As part of the pre-employment checks, should you be offered the role, we’ll carry out an Enhanced Disclosure and Barring Service (DBS) check, (unless you are currently registered with the DBS Update Service).

Salary range
