About the Role

This is a unique and rare opportunity for an experienced Occupational Therapist to join our small, friendly Home First Front Door Team (FDT). We are a fast-paced group dedicated to supporting complex hospital discharge planning in close collaboration with hospital teams and other partners and stakeholders across the county. We have an integral role within the transfer of care hub. You will have the opportunity to make a real difference, ensuring safe, timely and person-centred transitions from hospital to home or community settings.

The Front Door Team is a forward-thinking team that regularly leads and participates in innovative pilots and improvement projects, offering staff the opportunity to shape service delivery, test new approaches, and influence system-wide change.

The Home First Front Door team provides the link for people on their journey between hospital to home. The FDT team is part of Adult Social Care’s Home First service, which is made up of 5 neighbourhood teams (City, West, Vale, South and North). These teams provide reablement to support individuals to meet their goals and reduce their reliance on paid carers support. Additionally, the teams will support those people who have long term needs to be discharged safely from hospitals.

Your office base may be subject to change as we are currently transitioning between temporary and permanent office locations due to the closure of County Hall. If this is applicable to you, you will be notified, but no reimbursement will apply in respect of travel.
