Older Peoples Team West

Prevention of Admission Team
Hemel Hempstead
37 hours per week | Fixed Term

The Southwest Prevention of Admission Team supports people aged 65 and over, or those under 65 with dementia, who live at home in South and West Hertfordshire and are at risk of being admitted to hospital or residential care.

We provide same or next day support to help people stay safely at home and avoid admission. Our team helps when carers are unavailable or unwell, when someone’s health has worsened, or when short-term respite is needed. We also offer support to help people regain independence and reduce the need for longer-term care.

Our involvement is short-term and focuses on resolving urgent situations quickly and effectively.


Adult Safeguarding
Stevenage
37 hours per week | Permanent

Join our diverse and inclusive Adult Safeguarding team, to deliver our vision to the people of Hertfordshire, so that they feel well informed about what support available to them and how they can lead safe, happy, and healthier lives in a way they choose. This is an opportunity for change, and development of knowledge and skills in Safeguarding.


About the role

You will need to have knowledge and experience of relevant legislation.

You should be able to demonstrate a commitment to supporting people with the right care and information at the right time to help prevent future need, enable independence and promote positive wellbeing that will empower people to live the lives they want to within our Connected Lives Framework (Your connected life | Hertfordshire County Council).

You will be responding to those in need of social care support and undertaking the required work in accordance with the Care Act 2014 and Mental Capacity Act 2005.

We seek Senior CCO's who excel at engaging with people, promoting inclusive community options, and supporting a self-directed approach while maximising universal services. Our ideal candidates are resilient, robust, imaginative, and reflective. As a small team, we collaborate cohesively to achieve our outcomes, and we’re committed to smart and flexible working.


About you

Essential:

  • Paid or voluntary work experience in social or health care within the community
  • Full driving licence and ability to travel
  • An accredited RQF (registered qualification framework) level 4 social care qualification or equivalent. An equivalent would be a course that has aligned learning outcomes to the RQF level 4 social care qualification and involves an extended period of study with a supervised practice placement in a health, education or social care setting.

Desirable:

  • Knowledge and understanding of safeguarding inline with the Care Act 2014
  • Knowledge of other social care practices
