Team Manager – ASC Hub Outreach Team

Help shape the future of preventative Adult Social Care in Bournemouth, Christchurch and Poole.

The ASC Hub is the front door to Adult Social Care in BCP Council and brings together Contact, Outreach and Intake functions. The Outreach Team works with people who are new or not currently open to Adult Social Care, visiting people at home or in suitable community settings to understand what matters to them, identify risks and agree practical, outcome-focused solutions.

The team uses a preventative, strengths-based model to support independence and reduce avoidable demand on long-term services. This includes information and advice, signposting to community support, carers assessments, support with housing needs, low-level occupational therapy equipment, care technology advice and close working with housing, health and voluntary sector partners. The team also includes qualified Occupational Therapists, supporting a practical and specialist approach to helping residents remain independent at home.


Role Purpose

BCP Council is looking for an experienced and motivated Team Manager to lead our ASC Hub Outreach Team. This is a key leadership role within the front door of Adult Social Care, where you will help people access the right support at the right time and lead a team focused on prevention, independence and early intervention.

You will manage and support a team of Outreach Officers who work directly with residents, carers and partner agencies to identify practical solutions that help people remain safe, well and independent in their own homes and communities. The team provides timely visits, proportionate assessments, information and advice, signposting, carers support, low-level equipment, care technology advice and community-based support.

This is a chance to lead a service that makes a real difference earlier in people’s journeys, preventing, reducing or delaying the need for long-term care and support wherever possible. You will promote confident, strengths-based practice and support staff to make proportionate, risk-informed decisions that improve outcomes for residents and carers.

Main Responsibilities

As Team Manager, you will provide visible and supportive leadership, day-to-day management, reflective supervision and quality assurance for the Outreach Team. You will set clear expectations, support consistent practice and help the team work creatively with people’s strengths, networks and communities.

You will work closely with colleagues across Adult Social Care, health, housing and the voluntary and community sector to develop joined-up responses and avoid unnecessary escalation into more intensive services. You will also contribute to service improvement, performance monitoring and the continued development of the ASC Hub preventative model.

You will bring strong leadership, communication and organisational skills, and the ability to support staff through change while maintaining high standards of practice, safeguarding awareness and person-centred decision-making.


The Ideal Candidate

You will be professionally qualified and registered with Social Work England, the Health and Care Professions Council or the Nursing and Midwifery Council.

This is an opportunity to lead a committed team at an important point in the development of the ASC Hub. You will help shape how prevention and early intervention are delivered across BCP, supporting residents and carers to achieve better outcomes sooner.
