You will undertaking duties under the Care Act 2014 including assessments and reviews, work closely with social workers in the team to carry out capacity assessments, best interest decision-making, safety assessments, and provide interventions to maximise independence and wellbeing for the people in our service. You will work closely with other professionals and agencies and ensure people using our service receive an excellent standard of social care alongside their mental health care. You will work within key legislative frameworks such as the Care Act (2014), The Mental Health Act (1983) and Mental Capacity Act (2005). In this role, you will undertake a number of diverse specialist or technical activities for adults with care and support needs within organisational and legislative frameworks or assessments from other professionals. You will work closely with external partners and providers of social care reviewing care plans and care provision and responding accordingly to any Safeguarding concerns. Work is face paced but never boring, you will use skills that would be gained through formal qualifications and/or considerable relevant work experience. Roles at this level are expected to work unsupervised for much of the time, accessing and assessing information independently and can be undertaken by telephone, face to face in the community, office base or in service users home.Key Responsibilities:<ul><li>To work within the community Mental Health Recovery and Social Care Reviewing team to provided support and intervention for people with severe and enduring mental illness within the remit of adult social care within the integrated team. To work as part of MDT around the person to promote mental wellbeing, reduce and mitigate risk and promote positive outcomes for service uses.</li><li>To work with service users and families/carers to provide advice and support, assess risk and care needs of service users under the Care Act 2014 and S117 MHA to develop a framework for care plans/programme of intervention to meet their needs. We are looking to employ a flexible driver who are as passionate about our service as we. Community Meals drivers are more than a delivery person, they are in some instances the only contact our clients have with another person during their day. As part of the delivery service you will complete a welfare check, for example ensuring that the person is warm during winter months, or has sufficient drinks in warmer weather, reporting any concerns to the Business Support Team. We deliver our service for 363 days of the year, which includes weekends and Bank Holidays, pausing on Christmas Day and New Years Day.
