About the role…
As a MiCare Support Worker you:
<ul>
<li>Provide support to adults to develop and maintain activities of daily living, including personal care.</li>
<li>Support people to undertake a range of social care tasks such as washing, dressing/undressing, showering, and bathing, meal preparation, domestic activities and engaging in community activities.</li>
<li>Undertake a range of delegated health care tasks as required such as prompt prescribed medication, supporting the application of prescribed creams, eye and ear drops, catheter care and prompting exercise as prescribed by the physiotherapist.</li>
</ul>
It is essential that you have a full UK driving license and access to a car to enable you to work in this role.
Basic pay of £12.85 - £13.05 per hour, Saturdays at £19.28 - £19.59 per hour, Sundays and bank holidays at £25.70 - £26.11 per hour.&nbsp; 
Previous experience in a similar role would be advantageous, however, if you have the passion and commitment to help and support others then we can offer you a comprehensive induction and a tailored training plan, with support to undertake the QCF Diploma and Level 2 Health and Social Care qualifications fully funded by us!

Assessments need to be proactive, preventative, and personal and include support from other professional services where required, to ensure people receive the information and the level of support required for them to regain or maintain their level of independence and to be safe within the community.Provide goal setting / support plans for individual people as appropriate. Relief Needs Assessment Officer

About the roleWe’re seeking to recruit some great new Community Enablement Workers; people with the right values and positive attitude to work within the Specialist Support for Younger Adults team – could this be you?<ul><li>Our customers are aged between 18-65 and have a range of physical and learning needs.</li><li>We support them with personal care, manual handling, medication, food and drink, finances and shopping.</li></ul>We are looking for staff to work the following:<ul><li>Weekends only (Saturdays and Sundays every week between the hours of 7am and 11pm)</li><li>Day and Evening Shifts throughout the week between the hours of 7am and 11pm.</li></ul>We also have the below incentives:<ul><li>Enhanced rates of working from 8pm until 8pm including Saturdays, Sundays and Bank Holidays </li><li>Enhanced overtime pay </li><li>Business mileage paid at 45p an hour</li><li>28 days holiday, rising to 33 after 5 years of service.</li></ul>What will you be doing?<h4>Purpose of the job</h4>The purpose of this job is to provide high quality support and care to customers in their own homes within a wider community. To enable them to remain independent whilst supporting their wellbeing, rights and choices. To also support with any reablement needs and providing personal support where required.<h4>Principal responsibilities</h4><ol><li> To support people within their own home to live an independent life.</li><li>With reference to the individual’s care plan provide practical and emotional support to customers to help them maintain independent living. In appropriate cases:</li></ol><ul><li>Assist customers with physical disabilities to carry out activities, hygiene routines and manage medication as directed within their care plans to help maintain health and wellbeing;</li><li>Assist customers to undertake practical tasks to help maintain their physical wellbeing and/or improve their living conditions.</li></ul><ol><li>With reference to the individual’s care plan promote independence through the provision of information to customers and through liaison with carers, internal and external agencies. Part of your role will be to support people within their local community enabling them with volunteer placements, budgeting, social activities, and future planning of goals.</li><li>You will be responsible for updating daily record sheets, any communication with outside agencies and ensuring that any changes to the individual’s needs, are documented and reported to ensure that the care plan is a live document, whilst utilising departmental electronic record systems.</li><li>Undertake risk assessments, manual handling, and medication assessments in order to safeguard customers and staff, as well as to meet service standards.</li><li>You will be regularly reviewing the customers progress, monitoring the effectiveness and efficiency of the support plan whilst documenting adjustments where necessary. You will ensure that all changes within the persons care package are communicated back to managers.</li></ol>About you<ul><li>Satisfactory clearance of an enhanced Disclosure and Barring service check.</li><li>Educated to GCSE or Equivalent</li><li>Care Certificate (or willing to work towards)</li><li>Level 2 or above in Health and Social Care or equivalent (or willing to work towards)</li><li>Proven experience in health or care work</li><li>Experience of delivering care services in a community-based setting</li><li>Demonstrate an interest in working in this area and the ability to undertake the role</li><li>Good verbal and written communication skills.
