- Work with local communities to identify gaps, strengthen resilience, and support the development of groups, networks and community‑based initiatives. Maintain and update the relevant community resource databases.
- Commission innovative, proportionate care and support packages, aligned with what matters to the individual. Review and monitor plans in collaboration with the individual and provider.
- Analyse and respond to presenting risks and complexity, ensuring the right level of intervention.
- Recognise and respond to Adult and Children’s safeguarding concerns, acting in line with Part 7 of the SSWB Act and Wales Safeguarding Procedures.
- Work collaboratively with internal teams and external partners, including involvement in MARAC (Multi Agency Risk Assessment Conferences).

- Build strong, positive working relationships with partner agencies to enable coordinated, person‑centred intervention.
- Contribute to statutory responsibilities relating to MARAC processes.
- Commit to ongoing professional development, participating in training and working towards relevant qualifications where appropriate.
- Adhere to all departmental and local authority policies and procedures.
