Transitions and CHC Team Manager
Role: Team Manager – Transitions & Continuing Health Care
We’re excited to announce the launch of a brand-new, borough-wide social work team in Sefton! This team is dedicated to supporting young people as they transition from Children’s to Adults Services, and to assisting individuals applying for or receiving Continuing Health Care (CHC) and Joint Funding.
We’re looking for a passionate and experienced Team Manager to lead this important work. You’ll be supported every step of the way by a committed Service Manager and a passionate leadership team who are invested in your success.
As Team Manager, you’ll lead a dedicated team within Sefton’s Adult Social Care department. You’ll play a key role in shaping how we deliver services, working closely with health colleagues to develop integrated, person-centred approaches. You’ll have the autonomy to make impactful decisions and the support to grow your team creatively and effectively.
What does the role look like day to day?
We have recently created a new borough-wide social work team to support young people during their transition from Children’s to Adults services and support people applying for or receiving Continuing health and Joint funding.
We are looking for an enthusiastic Team manager to work with us to engage and inspire and be a key part of a dedicated team influencing and shaping how we deliver services for people.
In your role as Team Manager you will lead, support and manage a team within Sefton Adult Social Care Department as well as develop integrated approaches with health colleagues.
We are looking for a motivated and experienced individual to fill this role and bring passion, creativity and care to a really fantastic team seeking guidance, support and leadership. You should have an excellent understanding of the Care Act, associated legislation and Health and Joint funding processes. You will be responsible for key decision making and given the freedom to develop your staff and team creatively.
What we are looking For
We’re seeking a motivated and compassionate leader who can inspire and guide a team delivering high-quality adult social care. You’ll bring:
- A Social Work degree or equivalent qualification
- Registration with Social Work England (or equivalent health body)
- Management experience in Adult Social Care, ideally in Transitions and CHC
- Strong knowledge of the Care Act and related legislation
- Confidence in managing complex, person-centred decisions
- A commitment to equality, diversity, and inclusive practice
- Excellent partnership working skills with health and community stakeholders
- A passion for continuous learning and professional development

Salary range
