About the team

Are you a confident, forward-thinking leader ready to shape a pioneering social care initiative? The Prevention of Admission Team has an opportunity to take up the role as permanent Team Manager to lead a fast-paced, responsive service that supports individuals at risk of hospital, residential, or inpatient admission.

Launched in October 2023, this innovative service delivers rapid, community-based interventions to people aged 65 plus and/or those with a dementia diagnosis who are experiencing a breakdown in health or care circumstances. The team also provides critical support to carers in crisis, helping prevent carer breakdown, avoid emergency admissions to support people to remain safely in their own homes

About the role

As Team Manager, you will lead, motivate, nurture and manage a team of Social Workers, Community Care Officers to ensure the service provided is effective, collaborative and delivers positive outcomes within a 2hour response from referral to assessment across West Hertfordshire.

You will lead operational delivery, triage manage ensuring high-quality crisis assessments, and foster strong partnerships with health colleagues, particularly the co-located Central London Community Healthcare NHS Trust (CLCH) and their Rapid Response services.

  • Lead and manage the Prevention of Admission Team, ensuring timely and effective interventions
  • Drive service performance and uphold best practice in crisis assessment and care planning
  • Collaborate with Adult Care Services Teams, CLCH and other health partners to deliver integrated, person-centred care
  • Support staff development through supervision, coaching, and reflective practice

About you

Essential:

  • A qualified and registered social worker or occupational therapist
  • Full driving licence and access to a personal vehicle for travel across the locality
  • Proven experience in managing adult social care teams
  • Strong crisis management assessment and decision-making skills
