Job Title: Support Planner - CLS Team 4
Salary: £33,531 - £38,052 per annum
Grade: D
Contract Type: Permanent
Working Hours: 37 hours per week
Thurrock Adult Social care is seeking an exceptional individual to join us as a Support Planner in our Community Led Support Team 4. This role is an interesting opportunity to work in a Team that is committed to connecting individuals with their communities.
The team is about responsive social care support that creates opportunity for vulnerable people to come together with their community to explore opportunities and outcomes, while providing support that is committed to safeguarding the most vulnerable.
As part of a CLS team you will be working in a way that is responsive and proportionate, working with people to achieve their outcomes using their strengths and their community creativity, linking people with their communities focusing on ‘what matters rather than what’s the matter’. You will need to be an excellent communicator and able to work alongside colleagues in other departments, health, community partners and agencies in order to meet the care and support needs, and outcomes for our residents.
DBS Check: At Enhanced level with Adults Barred List is required for this post.
