We have an exciting, fixed-term opportunity for a Support Coordinator within our Early Intervention and Prevention team.

About you...

You will play a vital role in helping vulnerable people in Bracknell to stay out of hospital where possible, support them through safe and smooth discharges and empower them to live as independently as they can. You will play a key role in improving the lives of our residents and have a real impact on everything we do. Your voice will be heard, your ideas valued, and you will be trusted to take ownership of your work.

We are looking for someone who has experience of working within a social care or health setting and has knowledge of the Care ACT.

About the team...

We are always looking for better ways of helping our residents – how can we make the most of our resources? How can we get people back on their feet? This Support Coordinator role is valuable in making changes in people’s lives, supporting them and the rest of the team you will be working with.

You will be a valued member of our Early Intervention and Prevention team (EIP) - a close, hard working, fast paced and multidisciplinary team. Our team help people stay out of hospital where possible, help them when coming out of hospital if needed, to remain as independent as possible and have the support that they need. The work can be challenging but is very rewarding. You will receive regular supervision, access to training and opportunities to grow and develop in your role. We actively listen to our team, encourage feedback and value suggestions that help us improve the way we deliver services together.

Key responsibilities...

  • To provide advice, information, support, and care to vulnerable people in Bracknell.
  • To work with people to coordinate the identification of their needs and develop support plans to meet those needs, maximising choice, control and independence.
  • To case manage and support people on the Intermediate Care Service or receiving other short term services through the Early Intervention and Prevention Service.
