What you'll do

As a Social Worker Level 2, you will work with adults, families, carers and partner agencies to deliver strengths-based, person-centred support that promotes independence, wellbeing and positive outcomes.

You will manage a varied and often complex caseload, using your professional expertise to assess needs, develop effective support plans and empower individuals to achieve their goals. Working as part of a supportive multidisciplinary team, you will play a key role in safeguarding adults, promoting independence and improving quality of life.

Key responsibilities

• Manage a varied and complex caseload.
• Undertake Care Act assessments and strengths-based conversations to identify needs and desired outcomes.
• Develop, implement and review care and support plans that promote independence and wellbeing.
• Work collaboratively with health colleagues, community organisations and partner agencies to achieve positive outcomes.
• Support safeguarding enquiries and ensure adults remain at the centre of decision-making.
• Provide advice, support, coaching and mentoring to colleagues, contributing to a positive team culture.
• Maintain accurate records, reports and assessments in line with professional and legislative requirements.

You will be given a nominated contractual work base and you will be required to commute to this work base when necessary. Working arrangements regarding hybrid working will be discussed as part of the appointment process.

Our technology platform and equipment is very good enabling you to connect and collaborate remotely. We require that you have in place good connectivity, and we will discuss during the recruitment process if support with this is needed.
