We are excited to offer an opportunity to join our well-established Intensive Social Work Mental Health Team (ISWMHT), a skilled and dynamic service working alongside both our dedicated Approved Mental Health Professional (AMHP) Service & Inpatient Social Work Team and the Substance and Alcohol Team (SaAT).

This is an exciting time to join a team with an experienced workforce, innovative practice, and commitment to improving outcomes for adults experiencing serious mental illness. Our team is made up of knowledgeable and supportive Social Workers and AMHPs who bring a wealth of expertise, creating a collaborative environment where professional development and high-quality practice are actively encouraged.

The ISWMHT provides a countywide service to adults with complex and enduring mental health needs, delivering intensive, assertive outreach and relationship-based social work. We work alongside individuals, families, carers, health colleagues, and partner agencies to promote recovery, manage risk, reduce hospital admissions, and address the wider social determinants that impact mental health and wellbeing.

What you’ll be doing:

  • Managing your own caseload with regular supervision, support and development from Advanced Social Work Practitioners and your Team Manager.
  • Using your knowledge of the Care Act and statutory duties to deliver person-centred outcomes that promote independence and wellbeing.
  • Working as part of a supportive multi-disciplinary team alongside experienced Social Workers and AMHPs, with opportunities to develop your career and progress towards AMHP training.
  • Delivering strengths-based interventions that make a real difference to vulnerable adults through collaborative working, meaningful engagement and a focus on recovery and independence.

What you’ll bring:

  • Degree in Social Work & registered with Social Work England
  • Successfully completed the UK Skills for Care ASYE programme
  • It is an essential requirement of the role that the post holder has a valid UK driving licence and is either a car owner or can use a car for work purposes.
