The Role:
The Community Hospitals Team have an exciting opportunity for a Social Care Worker and are keen to hear from you. We are a large supportive team who are based primarily on the wards in the Community Hospitals and work alongside our health colleagues as well as our colleagues in the Acute Hospitals. The Team covers all the Community Hospitals across the county and is part of the wider Urgent Care Service, so we provide lots of opportunity to develop and gain experience. We also support with the Discharge to Assess model so also have some work in the wider community. I would like to give you an overview of a typical day but that is impossible due to the diversity.
As a Social Care Worker, you will work alongside the Social Work Team and be part of the Hospital MDT, building up strong working relationship with our Colleagues in the NHS. You will have your own case load but support from a Qualified Social Worker for more complex cases and none of us are far away.
As a Social Care Worker, you will be expected to have good listening skills, an enquiring mind and a determination to support adults to achieve their gaols and aspirations using strength-based discussions. We would expect you to have an interest in supporting individuals and their carers at what can be a very stressful time in their lives.
We would expect that you have some knowledge of the of the Mental Capacity Act, the Care Act be able to identify Adults who may be at Risk or Abuse. While we will always aim to limit the amount of travel, as a Countywide Service, there may be occasions where a flexible approach is required to support the wider service and Team Colleagues.
We have several Practice Educators within the Team so workers have plenty of opportunity to share their knowledge and learning and we pride ourselves on the support and camaraderie the Team provides. The Team meets regularly in Team Meetings where we have guest speakers and in smaller cluster meetings with ASWP’s where Workers have an opportunity to share knowledge, learning and aspects of good practice. You will have regular Supervision and support to ensure you can attend training any participate other aspects of learning.
