An opportunity has arisen within the Derbyshire Dales Mental Health Adult Care Team for a Social Care Practitioner.

The team works with people with mental health difficulties from the age of 18. You will require good working knowledge of the Care Act 2014 and Mental Capacity Act 2005, as you will be assessing clients care needs, and working with them and their support networks to create support plans. You will need knowledge of the Mental Health Act and other relevant legislation. Using person centred, strengths-based approaches you will be supporting people to live independently or within a more formal support setting.

A good working knowledge of Safeguarding is also essential.
