
Social Care Practitioner
An exciting opportunity has arisen for an enthusiastic and motivated Social Care Practitioner to join the Erewash Short Term Assessment and Reablement Team.
The Erewash Short Term Assessment and Reablement Team provides a multi-disciplinary approach to assess, support, and enable adults to maximise their independence and to assess for any potential long term care needs. We are committed to working in a person centred and strengths-based way, recognising peoples assets and strengths and enabling them to identify and reach their goals. The team are community based but will also support people leaving residential and nursing establishments and will include facilitating safe and timely discharges from some hospital settings. We are continuing to offer a hybrid approach to working, and our team is currently working from home and from the office based on the needs of the service.
The successful applicant will be able to demonstrate the necessary skills, knowledge and experience to undertake this role in order to provide a safe, accountable, cost effective and efficient service to our clients which helps them to achieve their outcomes. Key responsibilities will include carrying out assessments and reviews for individuals with less complex needs as well as arranging the provision of services to meet eligible needs. It is expected that you will be committed to working in a person centred and strengths-based way with clients and their families to maintain and maximise their independence. There is an expectation that you will act as duty worker on rota basis which will involve triaging new referrals to the team and support clients and carers who require a more urgent response. You will need to be able to build positive relationships with health partners and other agencies.

The job may require unsocial hours working including weekends and Bank Holidays. Payment arrangements will be detailed in any contract offer made.
Saturday and Sunday hours will be paid at plain time where it is part of your normal working pattern unless it is between 7pm and 7am, where enhanced rates of pay apply. Enhanced pay rates are paid for Bank Holiday working.
