Are you passionate about making a real difference in people’s lives? We’re looking for a dedicated Social Care Assessor to join our team and help shape personalised care and support within the community. In this role, you’ll be responsible for receiving referrals, completing holistic assessments, and supporting individuals through every step of their care journey — from planning and review to arranging essential equipment and adaptations.
You’ll play a key role in promoting choice, control, and independence, ensuring that every person you support has access to the care pathway that’s right for them. Working collaboratively with colleagues, partners, and the wider community, you’ll help influence and improve the local environment to enhance social inclusion, accessibility, and overall wellbeing.
Using established practice frameworks and standard operating procedures, you’ll contribute to the development of neighbourhood working, remaining flexible and responsive to the changing needs of the service. From time to time, you may also take on additional tasks aligned with your role to support the smooth delivery of services.

In this vital role, you’ll communicate effectively with service users, carers, and a wide range of professionals, promoting a strong, multi-disciplinary approach to care. You’ll carry out robust assessments, risk assessments, support plans, and reviews, while facilitating self-assessments where appropriate. Your clear and concise record-keeping and reporting will ensure high standards of practice, using council databases and systems to manage cases efficiently.
You’ll use your professional judgment to evaluate priorities, explore creative solutions, and design cost-effective care packages that deliver value for money. With a sound understanding of equality, diversity, and culturally competent practice, you’ll uphold the highest standards of personal and professional conduct — ensuring fair and inclusive service delivery at every stage.
We’re looking for someone with a basic understanding of social care legislation, including personalisation, eligibility criteria, safeguarding, MCA and DoLS, as well as welfare benefits advice. A willingness to learn and develop new skills — such as knowledge of occupational therapy equipment or minor adaptations — is essential.
You’ll hold a relevant qualification (or have equivalent experience) and be committed to your ongoing professional development. The role requires daily home visits and occasional work outside of standard office hours, as well as flexibility to work across different Adult Social Care Pathways.
This is a public facing role so you’ll need to show off your fluency in speaking and writing English.

Because of the nature of this job, it will be necessary for the appropriate level of criminal record disclosure to be undertaken.
