Job Title: Shared Lives Worker

Location: Hertfordshire

Salary: £34,434 per annum progressing to £37,280 per annum

Hours: 37 hours per week

Contract: 6 Months Fixed Term Contract/Secondment to December 2026

About the team:

We are currently looking to offer this exciting opportunity which has arisen in our Shared Lives Team; this service enables adults with a support need, to live in the local community within a family home. The role is a work anywhere role although will require travel to meeting venues as well as to Shared Lives carers homes.

As a Shared Lives worker, this is not a direct support role, you will provide professional support and monitoring of independent Shared Lives Carers contracted with Adult Care Services and work in partnership with operational teams, care managers and organisations within Hertfordshire and with other Local Authorities. Some experience of case work and knowledge of shared lives would be preferable.

About the job:

  • To manage a case load of Shared Lives carers who support adults with a support need within their home. Whilst taking responsibility for reviews, ensuring support plans are in place and updated support for adults with disabilities whilst taking responsibility for all practice and ongoing support planning, ensuring all staff are aware of and meet the requirements of the Care Quality Commission
  • Lead effective communication and liaison with Social Work Teams, Community Learning Disability Teams (CLDT), Health Professionals, other services, and transition teams working with young adults.
  • Developing and empowering shared lives carers to support adults with disabilities of which some will have other complex needs.
  • Responsible for advising on and developing policies, procedures, and operational systems to reflect the aims and objectives of the service.
  • Recruitment, Assessment and Supervision of Shared Lives Carers
  • Provide a professional response to identify support and assess prospective Shared Lives Carers.
  • Produce and present reports to the Shared Lives panel and provide additional information and professional view as required.
  • Provide regular support and monitoring of Shared Lives Carers as individual contractors for Shared Lives Hertfordshire.
  • Ensuring safe and effective support is provided in relation to safeguarding and risk management.
  • Promote the services offered by through networking and promotional events.
  • Make home visits to people interested in being shared lives carers and those interested in using the service.

About you:

Essential Criteria

  • RQF (Formerly QCF) Level 4 in Health and Social Care or equivalent.
  • Full driving license
  • Experience in a social care setting
  • Experience of working within regulatory body frameworks ie CQC or PAMMS
  • Experience working with adults with disabilities
