We are recruiting a Shared Lives Worker at Caerphilly County Borough Council.

Working hours: 37 hours
Contract Type: Permanent
Location: Working hours can be split between home and office depending on specific task requirements.

This is an exciting opportunity to join Team Caerphilly and provide support across the wider South East Wales Shared Lives Team.

We pay an attractive salary of £34,314 - £37,035 and offer access to training and development opportunities.

The South East Wales Shared Lives Scheme offers a unique community based service for adults who need care and / or support. Individuals are matched with compatible Shared Lives carers who offer to share their home, family & community lives.

The service offered is highly personalised where every individual has a strong voice, and opportunities are sought in consultation with each individual according to their personal preferences and chosen outcomes.

The service has a strong focus on supporting individuals to achieve the best possible outcomes.

The successful applicant will support the managers and coordinators to develop Shared Lives across seven local authority areas and Aneurin Bevan University Health Board (ABUHB). Recruiting and assessing new Shared Lives carers, working on referrals for adults with care and support needs, completing and reviewing personal plans, and setting up new Shared Lives arrangements.

The post holder will supervise and support a caseload of Shared Lives carers who provide arrangements in their own homes, this involves monitoring and reviewing the Shared Lives arrangements.

The post holder will work in close collaboration with other professionals and organisations, maintaining strong working relationships, contributing to the care planning process, and ensuring individuals are supported to achieve their personal outcomes.

For the role, we ask that you have:

  • A City and Guilds Level 4 Adult Placement/Shared Lives or past Level 3 qualifications on the Credit and Qualifications Framework for Wales
  • Knowledge of relevant legislation, guidance research and best practice
  • A Full UK Category B (Cars) driving licence and the use of a motor vehicle insured for business purposes to travel throughout the participating organisations to attend meetings, visit individuals and carers in their homes etc.
