Join Us in Transforming Care for Disabled Adults in Hertfordshire

We’re looking for a proactive Senior Service Finder to join our Community Commissioning for Disabled Adults team. This role is a key part of our mission to improve outcomes for people with complex needs across Hertfordshire.

You’ll be working closely with the Transforming Care Team—a vibrant, multi-disciplinary group committed to personalised care, timely transitions, and collaborative working across health and social care.

As a Senior Service Finder, you’ll support operational staff in the Adult Disability Teams and the 0-25 Together Service, helping to identify and secure person-centred housing and community-based services. Your work will directly impact the lives of people we support, helping them live more independently.

You’ll be part of a wider strategic commissioning team that spans Adult Disability, SpecialistMental Health, Children’s Services, and inpatient settings, and you’ll collaborate with both internal colleagues and external partners—including housing providers, care agencies, and health professionals.

About the role

This is a highly skilled and fast-paced role reporting to the Complex Care Commissioning and Monitoring Officer.

You will be responsible for identifying and commissioning services for individuals with complex hospital admission and discharge histories, as well as those at risk of admission.

About the role:

  • Managing a caseload of individuals aged 16+ with complex hospital admission and discharge histories, or those at risk of admission.
  • Identifying and commissioning bespoke, person-centred care packages, including costings and financial oversight.
  • Supporting discharge processes and representing the team in multi-agency meetings.
  • Liaising with providers to ensure quality support and negotiating care costs for individuals with Care Act eligible needs.
  • Attending inpatient reviews, Multi-Disciplinary Team meetings, and NHS England commissioner meetings.
  • Contributing to market shaping, service development, and milestone reporting.
  • Covering team duties during absences and managing urgent actions when needed.
  • Working closely with professionals across health and social care—including Adult Disability Services, 0–25 Together, Mental Health, Specialist Learning Disability Service, and integrated health commissioners.
  • Analysing internal data to support commissioning decisions and resolve financial queries.
  • Responding to complaints, managing conflict, and escalating issues appropriately.

About you

Essential:

  • Proven experience in both health and social care settings.
  • Full driving licence
  • Strong leadership, communication and project coordination skills.
  • Confident working with diverse teams, professionals, commissioners, and providers—able to challenge constructively.
  • Ability to interpret individual needs and translate them into service plans.
  • This role involves working across key HCC sites and in patient settings access to a vehicle and the ability to travel is essential.
  • Excellent organisational, IT, and time management skills.
  • Proficient in Microsoft Office and familiar with Liquidlogic systems (ACSIS).
  • Service development and provider engagement, with confidence in negotiating and influencing skills.
  • Data recording and presentation to support strategic planning.

Desirable:

  • Understanding of funding streams (e.g., s117 aftercare, waivers, client contributions).
  • Demonstrated ability to manage complex cases and multi-agency coordination.
